THIS FORM RELEASES THE STATE OF NEW MEXICO, EMNRD, STATE PARKS DIVISION, FROM ALL LIABILITY

• This waiver must be completed and signed before the TrackMaster wheelchair can be used.
• Users under 18 MUST be accompanied by a parent or guardian, and adult users must be accompanied by a companion.
• The TrackMaster wheelchair must be in the off position when getting in or out of the chair and for transfers.
• The State Parks Division requires users and companions to undergo instruction by the assigned State Parks Division staff before the TrackMaster wheelchair may be taken on a trail.
• Only the designated user who signed the waiver may be in the TrackMaster wheelchair.
• No other person can be on the lap of the designated user or ride on any other part of the TrackMaster wheelchair while it is in operation.

• I acknowledge I have been fully instructed on the use and limitations of the TrackMaster wheelchair.
• Assigned State Parks Division staff has described and physically shown me the uses and functions of the TrackMaster wheelchair.
• I understand the TrackMaster wheelchair can climb steeper inclines, obstacles, and variable terrain, but acknowledge that it can tip in any direction. The TrackMaster wheelchair has all-terrain rubber tracks and can move quickly and change direction abruptly while traversing obstacles or with any movement of the joystick. I have been trained on the operation and accept full risk.
• I assume all responsibility for my transfer in and out of the TrackMaster wheelchair.
• I agree to operate the TrackMaster wheelchair only on designated trails and only within the state park boundary.
• I acknowledge the TrackMaster wheelchair comes equipped with a safety belt and I will wear it at all times while operating the chair.
• I assume responsibility for my individual limitations while operating the TrackMaster wheelchair.
• I understand only one person may be on the TrackMaster wheelchair at a time, and that all persons operating the TrackMaster wheelchair must sign a waiver.
• I understand a companion/parent/guardian must always remain present with the TrackMaster wheelchair while I am using it.

Vehicles Per Site : ONE (1) vehicle is included in the online reservation fee. A motor home towing a vehicle or a vehicle towing a camping trailer is considered ONE (1) vehicle. Extra vehicles, including motorcycles, will be charged $15 per night per vehicle for New Mexico residents or $20 per night per vehicle for out-of-state residents, collected at the park. A Maximum of TWO (2) vehicles (A motor home towing a vehicle or a vehicle towing a camping trailer and one other vehicle) are allowed per site, space permitting.
PLEASE NOTE: When selecting customer type Volunteer Emeritus or NM Disabled Veterans Pass Holder you are required to show your qualifying documentation when arriving to the NM State Park where your reservations are booked. The non-discounted, full camping fee(s) will be accessed at the park without proper qualifying documentation.
